Purpose of the app

This app supports clear-aligner event intake, customer record creation, consent collection, dental and medical history collection, clinical photo capture, scan upload, checkout handoff, staff review, and manufacturer workflow preparation.

Not a substitute for professional advice

The app is not a substitute for professional dental, orthodontic, medical, or emergency advice, diagnosis, or treatment. Customers should consult licensed dental or medical professionals about clinical questions, symptoms, risks, treatment options, and whether aligner treatment is appropriate.

Not for emergencies

The app must not be used for emergencies or urgent dental or medical concerns. Patients with severe pain, swelling, trauma, infection concerns, bleeding, breathing difficulty, or other urgent symptoms should seek immediate care from a licensed professional or emergency service.

Evaluation and planning

Photos, scans, consent responses, and health history answers submitted through the app are used to support evaluation, planning, workflow review, and manufacturing handoff. They do not by themselves determine eligibility, final treatment approval, or the full scope of care.

Provider and manufacturer review

Clear-aligner treatment depends on appropriate review by qualified providers and/or treatment or manufacturing partners. Additional information, revised scans, different records, in-person evaluation, or referral may be required before treatment can proceed.

Treatment suitability and outcomes

Aligners may not be appropriate for everyone. Dental condition, oral health, medical history, compliance, case complexity, and provider judgment can affect suitability, timing, risks, and results. No result, timeline, approval, or outcome is guaranteed.

Minors and dependents

If information is submitted for a minor or dependent, a parent, legal guardian, or authorized representative should review all questions carefully, provide accurate information, and consult licensed professionals about treatment questions for that patient.
